Have a lab kit and need help? We can help you complete your kit and have it sent out for you. * When booking, pricing will vary depending on if you choose to come to our office or how far away you need us to travel for Home/Work visit: Office/Home/Work visit (0-25 miles)- $62.00 Home/Work visit (26-50 miles)- $82.00 *Ulta Lab clients, please book your service through either the Home/Work visit option or the Office option.

Cancellation Policy

In order to keep a proficient and punctual schedule, we have employed a 3 hour cancelation policy which states that any cancellations less than 3 hours from scheduled appointment will not be refundable. By proceeding with the scheduling process, it is assumed that this policy is understood and acceptable by all parties.
